Periodic Health Assessment Pha
Where do I have to get my PHA? (periodic Health assessment)?
I need to have my vision readiness checked, I need to update my immunizations, and my PHA. Can i get all of these done at any civilian clinic or does it have to be a clinic authorized by my unit?

You need to get a hold of your UA or squad leader. If you are in the reserves you can call 1-800-666-2883 to make appointments. If you have never done it before long back into your AKO you can filll out some information online.
