Aflac Wellness Benefit
Aflac hospital indemnity claim? How with labor/birth of baby?
I have a policy with Aflac insurance. I want to know exactly what forms to fill out to get the benefits with my Hospital Indemnity Policy and Wellness Policy. They sent me a Wellness form but I don’t think it’s the right one. Help. Having a baby Sunday/Monday.
I also have Aflac, and submitted my forms not too long ago; once you go to their site and click on “Claims Forms”, select your state and then select the form that says “Disability (Due to Sickness or Pregnancy). Fill out the applicable information, send it to your doctors office to fill out their portion, and then fax it to Aflac. It usually takes 5-7 days for them to review the claim, and then they’ll mail your check within a week of approval. Good luck with your delivery!
